Fortify Cares Program
Fortify has a team of Care Managers, Care Coordinators and Care Navigators whose goal is to help patients achieve their optimal level of wellness, inclusive of physical, emotional and social wellbeing. Our team works in partnership with pediatrician’s offices to provide additional support to families and children with certain chronic conditions.
